In addition to the Parent/Provider Contract, the following forms MUST be on file BEFORE your child's first day of attendance.

 

 

 

 

 

Parent Notification of Additional Children In Care (LIC 9150) This notifies you that I can have up to 6 children plus my two school-aged children.
